Annual in-service training and the agency records that go with it, for home health aides at a home health agency that participates in Medicare, structured against 42 CFR 484.80. 484.80(d) requires every home health aide to receive at least 12 hours of in-service training during each 12-month period. This pathway assigns the 12-course Home Health Aide 12-Hour Annual In-Service program (infection prevention, bloodborne pathogens, communication, observing and reporting changes in condition, emergencies, the needs of older adults, dementia, abuse and neglect, safe transfers, nutrition, skin changes, and personal care), which renews 12 months after each completion, and counts the aide's hours toward the 12-hour rule from the program's courses and the electives. IMPORTANT — what this pathway does and does not do: it supplies in-service CONTENT and the training RECORD (484.80(d)(2) requires the agency to maintain documentation that the standard is met). It does not, and cannot, discharge 484.80 on its own: in-service training must be supervised by a registered nurse (484.80(d)(1)), the competency evaluation must observe the aide performing listed tasks with a patient or pseudo-patient and be performed by a registered nurse (484.80(c)), and aides are supervised through RN visits (484.80(h)). All three remain the agency's responsibility and are tracked here as agency-held external evidence. No accreditation, approval, or continuing-education credit is claimed for this coursework.

Pathway curriculum
Each module shows the training or evidence that satisfies a requirement, when it is first due, and how often it must be renewed.
The 12-course in-service program, assigned once and renewed every 12 months from the aide's last completion. Covers the 484.80(b)(3) subject areas an annual in-service should refresh: infection prevention and control (iv) and a clean, safe environment (vi), bloodborne pathogens, communication (i), observing, reporting and documenting changes in condition (ii, v), emergencies (vii), the physical, emotional and developmental needs of older adults including dementia and abuse and neglect (viii), safe transfers (x), nutrition and fluids (xii), skin changes (xiii), and personal hygiene and grooming (ix). Complete when the aide finishes all 12 courses; the program certificate of completion carries the program title and the date. A new hire's first in-service year runs 12 months from the hire date.
Mapped training
42 CFR 484.80(d) requires every home health aide to receive at least 12 hours of in-service training during each 12-month period. This requirement counts the annual program above (13 credit hours once all 12 courses are complete) and each elective course, plus any in-service certificate with hours that the agency uploads (for example in-service the agency delivered itself), and is satisfied when the aide's hours inside the period total 12. The platform counts one credit hour per course material. TWO THINGS THIS REQUIREMENT DOES NOT DO: 484.80(d)(1) requires in-service training to be supervised by a registered nurse, and it may also occur while an aide is furnishing care, which only the agency can record. Both stay with the agency (see the RN supervision row below). A manager who checks this pathway out may map more products into this requirement to count in-service the agency delivers itself.
Applies to home health aides. 484.80(d)(1) allows in-service training to be offered by any organization but requires it to be SUPERVISED BY A REGISTERED NURSE, and 484.80(d)(2) requires the agency to keep documentation that the standard is met. This is the agency's obligation, not the platform's. Upload the RN's sign-off for the aide's in-service year (for example, the in-service record or attestation signed by the supervising RN) so the 12 hours above are evidenced as RN-supervised. Renews every 12 months with the in-service period.
Applies to home health aides. An aide may furnish home health services only after successfully completing a competency evaluation (484.80(c)). Communication (b)(3)(i), temperature, pulse and respiration (iii), personal hygiene and grooming (ix), safe transfers and ambulation (x), and range of motion and positioning (xi) must be evaluated by OBSERVING the aide perform the task with a patient or pseudo-patient, and the evaluation must be performed by a registered nurse (484.80(c)(1), (c)(3)). No online course can discharge that. Upload the aide's completed competency evaluation (the skills checklist signed by the RN) or training program record here.
Applies to home health aides. 484.80(h)(1)(iv) requires a registered nurse or other appropriate skilled professional to make an annual on-site visit to observe and assess each aide while the aide is performing care; for patients who receive no skilled services, 484.80(h)(2)(ii) requires that observation semi-annually (an agency whose aides serve such patients can set this row to 182 days in its checked-out copy). The 14-day and 60-day supervisory assessments of the services themselves (484.80(h)(1)(i), (h)(2)(i)) live in the patient record. Upload the RN's on-site supervisory visit note for this aide. Renews every 12 months.
